Please see the conditions contract of donation (agreement of accession) and with the list of documents necessary for consideration of your request (about payment of treatment, operation, medicines or means of rehabilitation):

Important: it is necessary to describe in detail our problem, so that we can acquaint with it conscious and not indifferent fellow citizens. People always want to have the fullest possible idea of your misfortune and how to help. The letter must be signed.

a copy of the author's passport & nbsp; (pages with a photo and residence permit);
certificates of income from the place of your work, and for minors from the place of work of parents (official representatives), or the conclusion of social protection agencies on the financial situation of the family;
a copy of the child's birth certificate;
a copy of the last medical report with a diagnosis, on the medical organization's letterhead, with the doctor's signature and stamp;
a bill on medical treatment issued by a medical institution, preferably with a full list of services and medicines and bank details;
a copy of the application and refusal of assistance from the social protection authorities, the Ministry of Health of the Russian Federation or the region (requests for payment of medical assistance, rehabilitation funds, medicines);
Your color photographs or color photos of the child (at least five different: large, medium-sized, footage from life, not from documents);

if you are a guardian of a child, a copy of the decision of the guardianship authority for the transfer of the child or a copy of the certificate of guardianship is required.

The letter and documents must be sent by regular mail or to the e-mail address of the Foundation.

Attention! Requirements for documents sent by e-mail:

text files are accepted in pdf or jpeg format, photos in jpeg, tiff or png;
one document in a separate file;
the size of one file should not exceed 1 MB;
the total amount of investments in one letter should not exceed 7 MB;
links to file sharing and files compressed into the archive, we do not accept.

The final decision on the publication of your materials is taken by the Foundation's expert council. We will notify you of his decision or the need to provide additional documents.

If you decide to thank those who helped you, your letter of gratitude will be published on the website.
